COSI 2.0 is an AI-powered digital tool designed to help patients identify their communication needs and track the benefits of hearing aids over time. Built on the foundations of a trusted clinical tool, it brings together client empowerment, clinical efficiency, and data-driven insights in a user-friendly digital experience.
What is COSI?
The Client Oriented Scale of Improvement (COSIâ„¢) was developed by the National Acoustic Laboratories (NAL) in 1997 as a paper-based tool to measure communication improvement based on goals set by the patient. It has since become the gold standard for personalized hearing care, helping clinicians understand individual needs and measure real-world outcomes after hearing aid fitting.
COSI 2.0 builds on this foundation—modernizing the process with AI and automation to make the experience faster, easier, and more insightful for both patients and clinicians.
How does it work?
Before their appointment, patients receive a personalized communication needs assessment via text or email. This guided process uses AI to help them reflect on and articulate the specific communication challenges they’d like to improve. After receiving hearing technology, they’re invited to complete a follow-up assessment to track how well their hearing aids are addressing those needs.
The process is fully automated, including reminders, and the results are instantly accessible to clinicians—allowing them to prepare more effectively and focus on personalized care during the appointment.

Key Features

The original COSI was introduced in 1997 and has been widely adopted in clinical best practice. COSI 2.0 retains its core strengths while enhancing usability, consistency, and scalability through digital innovation.
